EFICACY OF POST –OPERATIVE PAIN RELIEF MEASURES AND PATINET SATISFACTION WITH CONTINUOUS ANALEGESIA VERSUS INTERMITTENT ANALGESIA

Abstract

The purpose of the study was to determine the efficacy of pain relief and patient satisfaction with continuous analgesia (epidural with bupivacaine 0.1% and fentanyl 2mcg/ml at the rate of 10 ml/hour and patient controlled analgesia with morphine 1 mg/ml at the rate of 1 ml/hour, demand 1mg/hour with lock out interval 120 minutes, intravenous) and intermittent analgesia (Narcotic Pethidine intramuscular 1mg/kg body weight four hourly first 24 hours, six hourly next 24 hours.).

Gate theory of pain by Melzack and Wall (1965) was essential to understand the concepts pain and its management. The conceptual frame work was derived from General Systems theory by Ludwig Von Bertalanffy (1968).

Methodology

The research design was an experimental design with a study group and control group. The sample size was 50; 25 in each group. Sample selection was done by probability sampling method using simple random sampling – random numbers table

The tools consisted of a questionnaire on demographic data, observation checklist with a four point scale for scoring pain, sedation nausea and vomiting, vital signs and other complications for a period of 48 hours post-operatively starting from the time the patient reached the ward, pain rating using VAS with verbal anchors – a ten point scale ranging from no pain to unbearable (worst pain) which was administered at the end of 48 hours post operatively and an interview schedule to assess the patient satisfaction with pain relief measures using a four point likert scale at the end of 48 hours post operatively and a self administered questionnaire on Nurses’ opinion with multiple choice to assess their opinion regarding the above pain relief measures among surgical nurses towards the end of the study. Training was given for all the surgical nurses to monitor the above patients.

Ethical considerations were taken into account, informed consent obtained from subjects and confidentiality maintained.

Results

The major findings of the study were the overall pain relief (mean=27.08, SD=3.33) and the overall sedation (mean=21.08, SD=3.11) were better with continuous analgesia compared to overall pain relief (man=22.28), SD=4.38) and overall sedation (mean=18.44, SD=3.88) with intermittent analgesia. There was significant difference in pain relief and sedation between the groups (p<0.05). There was no significant difference in nausea and vomiting between the groups (p>0.05), there was significant association between the patient’s perceptions of pain using visual analog scale at the end of 48 hours post operatively and the groups(p<0.001), levels of satisfaction were higher with continuous analgesia ( mean for pain=2.4, mean for sleep=2.12) compared to intermittent analgesia(mean for pain=1.36, mean for sleep=1.36) and majority (80%) of the nurses preferred continuous analgesia as pain relief measure for their patient.

Conclusion

In this study the investigator concluded that the study group which received continuous analgesia had better pain relief and patient satisfaction compared to the control group which received intermittent intra muscular narcotic. The research hypothesis continuous analgesia provides better pain relief and patient satisfaction was accepted. The use of pain assessment and planned intervention could help for better patient care. Continuous analgesia can be routinely practised. Nurses can play a pivotal role in managing acute pain by improving the quality of pain assessment and pain management in post operative patients.

A STUDY TO EVALUATE THE EFFECTIVENESS OF COPE(CREATING PROGRAMME FOR PARENT EMPOWERMENT ) PROGRAMME ON PARENTAL AMXIETY IN PARENTS OF HOSPITALIZED CHILDREN IN PEDIATRIC INTENSIVE CARE UNIT (PICU) OF A SELECTED HOSPITAL IN DELHI

Abstract

A study to evaluate the effectiveness of ‘COPE’ (creating opportunity for parent empowerment) programme on parental anxiety whose children were admitted in paediatric intensive care unit of a selected government hospital of Delhi, was conducted by Miss Josy A Mathew at Rajkumari Amrit Kaur College of Nursing, University of Delhi, during the year 2002-2003.

The objective of the study were

a. To assess and evaluate the anxiety level of parents of hospitalized children in PICU before and after COPE programme

b. To determine the relationship between parental anxiety and selected variables.

  • Parental variables :Age, sex, socio-economic status, previous experience of hospitalization in PICU, support-system and trait anxiety.
  • Child variables : Age, sex, birth order and severity of illness of child
Methodology

The conceptual framework adopted for the study was developed from the ideas drawn from the Aguilera and Messick’s (1990) model of balancing factors in the stressful situation.

The research approach adopted for the study was experimental with pre-test post-test control group design. The study was conducted at Kalawati Saran Children’s Hospital, New Delhi. Purposive sampling technique and random assignment were used to obtain an adequate size of sample. The sample comprised of 30 parents in each group whose children were admitted in paediatric intensive care unit.

Speilberger’s state and trait anxiety inventory was used to assess the parental anxiety. Risk-severity scale, family support scale Kuppuswamy’s socio-economic status scale and observation checklist were used for collecting data on selected variables. The content validity of the tools and COPE programme was established by 11 experts in the fields of nursing, pediatric medicine and psychology. The COPE programme comprised of 3 strategies such as orientation of parents to PICU environment, facilitate the parents to verbalize their feelings and concerns and distribution of an information booklet.

Results

Data were collected from 18th December 2002 to 30th January 2003. The data obtained were analyzed, using both descriptive and inferential statistics. There were two parents out of 60 who had mild anxiety in pretest of both the groups. The others had moderate and severe anxiety. It indicates that parental anxiety is high when the children are admitted in intensive care unit. The mean post test anxiety scores of experimental group was lower than that of mean pre-test anxiety scores of experimental group. The mean post-test anxiety scores of experimental group was lower than that mean post-test anxiety scores of control group. It indicates that COPE programme was effective in reducing parental anxiety.

Findings also reveal that variables like parental age, sex and child’s age, sex and birth order had no significant relationship with parental anxiety. Parents with high support system, high socio-economic status, previous experience in PICU and low trait anxiety experienced significant reduction in their state anxiety.

Convenient sampling was used to select the hospitals in Mangalore. 40 mothers were selected by using purposive sampling method. The tools used wasa structured knowledge questionnaire to assess the knowledge of post operative self care among mothers undergoing elective caesarean section. Data was analyzed using descriptive statistics (frequency distribution, mean and standard deviation) and inferential statistics ( paired t-test and chi square test).

Results

The collected data were analyzed by using descriptive and inferential statistics. There was significant difference found between the mean pre test knowledge score and mean post test knowledge score,(‘t’ calculated value of pretest and post test knowledge scores=18.000, p <0.001) which showed that self instructional module was effective in improving the knowledge of mothers on post operative self care after caesarean section.

There was significant association between the level of knowledge and demographic variables namely age, parity, education, occupation, monthly income, exposure to health awareness, and history of caesarean section.

Conclusion

Findings of the study revealed that majority of the mothers had average knowledge. The self instructional module helped them to improve their knowledge on post operative self care after caesarean section. Hence self instructional module was found to be effective in educating the mothers on post operative self care after caesarean section.

A STUDY TO EVALUATE THE EFFECTIVNESS OF TRAINING PROGRAMME ON HOMECARE MANAGEMENT OF NEUROLOGICALLY DEBILITATED CLIENTS ON THE SELT EFFICACY OF THEIR CARE GIVERSIN SELECTED WARDS AIMS KOCHI

Abstract

A quasi experimental study was conducted to evaluate the effectiveness of training programme on home care management of neurologically debilitated clients on the self efficacy of their care givers at Amrita institute of Medical Science, Kochi.

Objectives of the Study
  • compare the self efficacy of care givers of neurologically debilitated clients before and after training on homecare management.
  • assess the occurrence of complications among neurologically debilitated clients after training on home care management for their care givers.
  • identify the association between self efficacy and selected demographic variables.
Methodology

A quasi – experimental pre test – post test design was adapted for the study. Convenience sampling was used to select 30 care givers of neurologically debilitated clients who were admitted in the Neuro medical and surgical wards of AIMS, Kochi. The tools used were a structured questionnaire to assess the knowledge of care givers, check lists to assess the skill and a self report questionnaire to assess the occurrence of complications in the clients by conducting a telephonic interview with their care givers. Data was analyzed using descriptive statistics (frequency distribution, mean and standard deviation) and inferential statistics ( paired t-test and ANOVA test).

Results

There was significant improvement of knowledge and skill of care givers in all the content areas after the training programme and the difference in means was highly significant (p= 0.001). Among the complications related to poor oral hygiene 2(6.9%) clients had halitosis, 1(3.4%) client had plague, and 1(3.4%) client had gingivitis. None of the clients reported any signs of pressure ulcer and only 2 (6.9%) clients had skin problems. Diarrhea was the commonest 3(10.3%) among gastro intestinal complications, 2(6.9%) clients had constipation and 1(3.4%) client had vomiting. Regarding the respiratory complications 2(6.8%) clients had fever, 3(10.3%) clients had experienced at least one episode of cough, and 1(3.4%) client had dyspnoea. The ANOVA values computed between self efficacy and selected demographic variables found that there was significant association between post knowledge score with age, education and occupation at 0.05 level of significance.

Conclusion

Based on the findings of the study the following conclusions can be drawn. The training programme on home care management is effective in improving the self efficacy of care givers of neurologically debilitated clients. The low incidence of complications among the clients, when compared to the previous findings obtained from the survey results regarding these complications, reveal that they have received a quality care. Hence this effective training programme can be implemented as a way of improving self efficacy of care givers and enhancing the quality of life of the client after discharge.

A STUDY TO FIND EFFECTIVENESS OF SKIN TAP TECHNIQUE ON PAIN RESPONSE DURING DPT INJECTION AMONG INFANTS IN VARIOUS RMCA CENTRES OF KNC MANIPAL

Abstract

Skin tapping is an effective technique for reduction of pain response during injection. The present study used this technique during DPT injection.

Methodology

A post test only control group design was adopted for the study. The sampling design was purposive sampling with random allocation of treatment using chit method with non replacement technique. The sample size was sixty; thirty each in experimental and control group.

Results

The study revealed that the pain response was less in experimental group. Majority, i.e. 24 (80%) of the infants in experimental group had mild pain whereas only 5(16.66%) of the infants in control group experienced mild pain. Independent t test was done to establish the effectiveness of skin tap technique. The t value was found to be 7.401 at p<0.001. It also revealed the association between the pain scores and selected variables like gender and weight of the child. The χ2 value for gender was 0.033 and weight was 3.032 in experimental group while it was 1.356 for gender and 9.710for weight in control group.

Conclusion

The study concluded that the pain scores in experimental group was independent of the selected variables such as gender and weight, while gender was independent and weight was dependent in control group.
